Managing medications from hospital to home: Can we do better?
 
Our May café was the first out of our two discussions on medication reconciliation. Healthcare professionals and the general public were part of our audience. They asked many relevant questions about different aspects of medication management, medication complications and the protection of patient data. The discussion covered most aspects of prescription management, from obtaining the best possible medication history, new electronic tools available to prepare prescriptions and confidentiality issues.
Our panel speakers Dr. Ewa Sidorowicz, Director of Professional Services for the MUHC, Dr. Allen Huang, geriatrician at the Royal Victoria Hospital and researcher at the RI-MUHC and Dr. Christian Rochefort, registered nurse and researcher at the RI-MUHC, all delivered compelling and accessible presentations about their research and clinical experiences. Dr. Ari Meguerditchian, surgical oncologist at the Royal Victoria Hospital and researcher at the RI-MUHC did an excellent job moderating the discussion.
